 BASIC REQUIREMENTS:


A. New Establishments
• Barangay Clearance to operate the business from where the new business establishment is located
• Community Tax Certificate (sedula)
• Phil-Health Registration (if not a member) and /or monthly contribution receipt (if member)
• SSS ID w/ monthly contribution receipt & Certificate of Compliance (SSS Office)

 Clearances/Certifications-  MUNICIPAL HALL
1. Mun. Health Office (Sanitary Permit)
2. Mun. Planning & Development Office (MPDC Office)
3. Mun.  Environment & Natural Resources Office (MENRO)
4. Mun. Engineer’s Office/Building Official (Engineering Office)
5. Mun. Assessor’s Office (Real Property Tax Clearance)
6. Bureau of Fire Protection

• Other Requirements:
• For Corporation: SEC Registration
• For Single Proprietorship: DTI Registration (5 years validity)
• For Cooperative: CDA Registration,
• For General Engineering Business: Contractor’s License
• For Selling of Drugs & Related business; also Food Processing like Bakery: BFAD License to Operate
• For Banks & Pawnshops: Bangko Sentral Ng Pilipinas registration
• For Retailer/Dealer of Lumber, copra, whole nut, charcoal, lambanog and other Coco- by products, (Miscellaneous Trees): DENR Clearance
• For Retailer/Dealer of Coco Lumber: PCA Registration from Coconut Development Office
• For Public Market and Other Economic Enterprise Stallholders: Contract of Lease

B. Renewal
• Barangay Clearance to operate the business from where the business establishment is located
• Community Tax Certificate (sedula)
• Previous Year’s Business Permit
• Phil-Health Registration (if not a member) and/ or monthly contribution receipt (if member)
• SSS ID w/ monthly contribution receipt & Certificate of Compliance (SSS Office)
